Managing HIT with Orgaran

Once switched from heparin to Orgaran, a patient's platelet count may rise rapidly, however25:
- a risk of thrombotic complications remain for approximately 5-6 weeks due to the presence of HIT antibodies therefore:
- prolonged parenteral anticoagulation may be required.
- Orgaran should be administered intravenously for the treatment of HIT26.
- Once in use, only individuals over the weight of 90kg need routine monitoring (amidolytic assay is recommended)26.
- Orgaran is recommended by the American College of Chest Physicians (ACCP grade 1B) as an alternative antithrombotic in isolated HIT18.(see table 1 below for dosing schedule)
Orgaran is an anticoagulant, similar in structure to heparin. Orgaran can be used as a substitute to heparin in patients those who have a previous history of HIT who require treatment for thrombotic disorders24.
